News and Important Information
The Higher Education Opportunity Act will go into effect July 2010.
This law will require textbook orders to be turned in prior to students registering for class. Please make certain that you get your book orders in to Christine in the Math and Statistics office by the published deadlines so that we will not risk being in violation of the law once it is effect.

Annual Activity Reports
The due date for Annual Activity Reports is: TBA
Instructions for MS Word format: Click and open file. Enter your information for the different sections by placing your cursor in the box and clicking once. The box will expand to fit. Save and print your completed report and bring it to Vonn Wyrick in Room 120 MM by the above deadline.
Instructions for .txt format: Click and open file. Place your cursor below the description and click once. Enter your information and point and click to move the cursor to the next section. Save and print your completed report and bring it to Vonn Wyrick in Room 120 MM by the above deadline.

In order to download the forms listed below you will need to have Adobe Acrobat or Acrobat Reader installed on your computer. You can download the Adobe Acrobat Reader program here.
Form instructions: Click on the highlight field icon to turn the fill-in fields light blue. Select a field by clicking on it with your mouse and enter the necessary information. When the form is completed, print the form.
You may also just print out the blank form.
NOTE: The completed forms cannot be saved to your computer. Only the blank form will be saved, not the entered data.
CoSM Special Projects Fund
To get money from CoSM's Special Projects fund for student travel, multidisciplinary lectures, and seminars by women speakers you need to fill out the following forms and turn them in to the Math & Statistics office. See here for information on the amount of payment permitted for each type of event.

Textbook Order Forms
